To make an appointment, call (818) 788-4220 and speak with our receptionist to find a mutually convenient time. She will answer any questions you have and send our intake form to your computer or smartphone. Fill it out securely online and click the link to submit your information. If you don’t have access to a computer, please arrive 15 minutes early for your appointment.

Payment and Insurance Policies

To offer quality service at a reasonable cost, we expect payment at the time services are rendered. We accept cash, checks, Visa and MasterCard. We will provide you with an itemized bill to attach to your insurance claim form. We do not accept MediCal. For further information, read on.

Private insurance
While most private insurers cover acupuncture and chiropractic, Dr. Wells is not a participating provider under any private insurance plans. Please check your policy to find out if, and to what extent, Dr. Wells’ services are reimbursable as a non-participating provider.

Medicare
We are non-participating Medicare providers. This means if you are eligible for Medicare, you pay us in full at the time service is rendered. Medicare may reimburse a portion of the chiropractic adjusting fee. However, they do not cover any other services we provide at Wells Chiropractic.

Automobile insurance
We may bill your automobile “medpay” insurance once we have verified benefits by phone. Prior to verification, payment is expected at the time service is rendered. We do not accept attorney liens, and we do not bill third parties (the person that hit you). You are responsible for any amount not reimbursed by your auto carrier.

Workers’ Compensation insurance
We do not accept workers’ compensation cases. If you wish to be treated for a work-related injury, you may do so at your own expense.

Personal Injury
If you have a slip and fall injury, you are responsible for your own care. We will provide a detailed statement that you can submit for reimbursement to the “at fault” party.
